You can join if you have type 1 or type 2 diabetes and your employer, health plan or healthcare provider offers the program.

• Eligibility is based on your benefit sponsor (employer, insurer or provider).
• Enrollment is simple: You’ll be guided through the process once you confirm your eligibility.
• Covered by many of the largest in-network health plans—including Blue Cross Blue Shield, Centene, Elevance Health and UnitedHealthcare. Services are based on your individual coverage. Learn more about your eligibility by completing the registration process.

After enrolling, you’ll get a Welcome Kit delivered straight to your door and immediate access to the program.

Only you, your Teladoc Health team, and other people involved in your care and care coordination can see your readings.

• Your employer or health plan cannot view your individual results
• You can request that Teladoc Health share your readings directly with your doctor

Your Teladoc Health coach or provider can recommend what’s best for you, but most members check daily to track trends.

• Checking regularly helps you and your team see patterns and progress

Maybe—the program currently provides a connected blood glucose meter with unlimited test strips and lancets, but may be able to accommodate a CGM.

• If you already use a CGM, you can discuss it with your provider to see how it fits with your plan
