What is the Diabetes Program?
The Diabetes Program helps make living with diabetes easier by providing you with a connected meter, unlimited strips, and coaching.
Who’s eligible for the Diabetes Program?
Our program is available to people with a diagnosis of type 1 or type 2 diabetes. It is available at no cost through partnerships we have with particular employers, health networks, or benefit programs. For specific employee coverage requests or inquiries, please check with your HR manager, benefits coordinator, or you can email membersupport@teladochealth.com and we can check for you.
What should I expect after registration?
After enrollment, you will receive a welcome kit within 5 business days. Your welcome kit will include the following:
- A blood glucose meter
- Test strips
- Lancets
- Lancing device
- Control solution
- USB cable (charger)
- Carrying case
Once you receive your Welcome Kit, please read the quick-start guide located inside the box prior to using the device.
How do I cancel my membership?
You can cancel your membership at any time. To unenroll, call us at (800) 945-4355. You can also email us at membersupport@teladochealth.com.
