Get a doctor’s note

If you need a doctor’s note for work, school, or another obligation, here’s how the process works and what to expect.

Requesting a doctor’s note during your visit

You can ask your provider for a doctor’s note during your appointment. Approval is based on the provider’s clinical judgment and is not guaranteed.

If approved, your note will be available after your visit in your account.

Where to find your doctor’s note

Your excuse note will appear if it was approved by your provider during the visit. 

To access your note:

  1. Log in to your account.
  • On a computer: 
    • Select "Messages" from the top
    • Select the correct provider   
    • Select "Excuse note" to download your note
  • From the app
    • Select "Messages" from the bottom    
    • Select "Care team"     
    • Select the correct provider
    • Select "Excuse note" to download your note

If you didn’t receive a note

If your note contains incorrect information such as:

  • Misspelled name
  • Incorrect dates
  • Incorrect free-text details

Please contact support within 72 hours of your completed visit so we can review the issue.

After 72 hours, corrections cannot be made.

How many days does a note cover?

Typical doctor’s note timeframes are:

  • General Medical visits: 1–3 days
  • Mental Health visits: 1–7 days

Additional days are not guaranteed and must be reviewed within 72 hours of the visit.

Important things to know

Notes cannot cover dates before your visit

Providers cannot issue notes for dates before the appointment took place.

Return-to-work or return-to-school notes

Providers do not issue formal return-to-work or return-to-school clearances. Employers and schools may use your doctor’s note at their discretion.

Provider signature

Doctor’s notes include the provider’s name, but not a handwritten signature.

Membership status

Doctor’s notes are available only to active members. If your membership has ended, you may still request your medical records through our privacy request process.
