The Diabetes Prevention Program helps people lower their risk of developing type 2 diabetes with connected tools, lifestyle support and personalized coaching.

• Members receive a connected scale that uploads weight readings automatically to the app
• The program includes guidance on nutrition, activity and healthy habits
• Everything is delivered at no cost for eligible members

You can join if you are eligible through your employer, health plan or healthcare provider and meet the program criteria.

• Members who already have diabetes do not qualify.
• Members who are pregnant or have a BMI that is too low may not qualify.
• Spouses and dependents may also be eligible depending on the benefit.
• If you’re unsure about eligibility, you can check online or contact support.
• Covered by many of the largest in-network health plans—including Blue Cross Blue Shield, Centene, Elevance Health and UnitedHealthcare. Services are based on your individual coverage. Learn more about your eligibility by completing the registration process.

After you enroll, you will receive a Welcome Kit with the tools you need to begin.

• The kit includes the connected scale and instructions
• You can download the Teladoc Health app to track progress and explore coaching features
• Once your scale is activated, your readings upload automatically so you can follow your progress

You do not have to use the app, but it helps you get more out of the program.

• The app makes it easy to log food, activity and weight
• You can view trends and receive guidance from coaches
• The app connects your daily habits to your long-term goals

Only you, your Teladoc Health team, and other people involved in your care and care coordination can see your readings.

• Your employer or health plan cannot view your individual results.
• You can request that Teladoc Health share your readings directly with your doctor.

Most members weigh themselves daily or several times a week, but your coach can recommend what works best for you.
