Getting started with your continuous glucose monitor*

Using a continuous glucose monitor as part of the Diabetes Management program gives you the tools you need to take control of your health. Our program goes beyond just displaying numbers and charts. It provides you with insights and support to help you make informed decisions about your well-being. Here is how to get started: 

Follow these six steps for success: 

  1. Sync or activate your CGM device. Under Today’s Tasks, new members must activate their device. Existing members select Sync your CGM. 
  2. Connect your CGM device. Choose your supported device from the list of options. Sign in with your CGM account. 
  3. Go to the glucose data page. Once connected, go to the glucose data page. It may take up to three hours for some data to appear. This includes average glucose, time in range, time below range and time above range. And don’t forget, there is at least a one-hour delay in your glucose data.
  4. Review the glucose data page. The glucose data page provides you with a daily, seven-day and 14-day overview of your glucose readings. At the top of the screen, you’ll see a date range (e.g., past 24 hours, June 1-7). This reflects the time and date range of the data shown. The glucose data page includes your blood glucose meter (BGM) checks if you’ve been using your device. It also features helpful quick links and a list of synced devices and apps.
  5. Continue to visit the glucose data page. Once your CGM is connected, you can access this page from the app’s home page or the Condition Management dashboard. Every time you view the glucose data page, your info updates. This means you’ll receive new insights, charts and data each time. The data syncs with at least an hour delay. Do not use it to make treatment decisions.
  6. Connect with a coach. Your program offers unlimited access to a health coach at no additional cost to you. Your coach is a certified diabetes educator. They can offer personalized guidance throughout your health journey. Your coach can help you set realistic goals. And they can offer feedback on your eating patterns when you track your food in the app. They’ll teach you ways to improve your mindset and build confidence. You can use the app to message a coach or schedule a session with them anytime.

How to use the glucose data page 

The glucose data page puts everything you need in one place. Let’s break down the different features on this page. 

Navigation 

You can navigate to the glucose data page by using the daily, seven-day and 14-day tabs at the top of the screen. Each section provides an overview of that period. The exact dates of these time frames are shown just below. If you have data from wearing an Abbott or Dexcom CGM prior to connecting with the Teladoc Health app, you will be able to view your data for up to 180 days in the past.  

Overview data 

The overview data at the top of the page includes four main metrics across the daily, seven-day and 14-day views. The metrics include average glucose, time in range, time below range and time above range. The 14-day view also includes the glucose management indicator (GMI). This section updates every time you visit the glucose page (with at least a one-hour delay). To better understand what these numbers mean, check out this article

Insights 

Insights interpret your CGM data. They can help you find patterns and trends and see how those might relate to your daily habits. You’ll receive new daily, seven-day and 14-day insights every time you’re in the app. Insights help educate and encourage you. They provide actionable information to help you better understand your health and take steps forward. Tracking your food and syncing your steps will unlock even more personalized insights. 

Charts 

The charts are another way to interpret your data. The goal is to depict the possible cause and effect between your lifestyle and glucose levels. The daily chart reflects your glucose readings over a 24-hour period. The seven-day chart reflects your seven-day glucose average and range across four time periods in a day. The 14-day chart reflects the same as the seven-day but over a 14-day period. 

Blood glucose monitor (BGM) checks 

The glucose data page highlights your blood glucose monitor (BGM) checks. The daily view shows a time stamp of when you last checked using your BGM device. It displays the number of checks, number of checks in range, and the number of highs and lows over that period.  

The seven- and 14-day views include the average number of checks per day as well as the total number of checks in range. The views provide the total number of highs and lows. They also include a chart showing daily average glucose levels for that time frame. 

Quick links 

On the glucose data page, you’ll also find links to your BGM logs and health summary report. There is a link to easily track food and a link to an article explaining what your data means. Tracking your food and steps will give you more personalized insights. You’ll begin to understand how food and activity affect your glucose levels and overall trends and patterns. 

Devices and apps 

Any apps or Teladoc Health devices that you’ve synced with the app are listed on this page. You can sync any Teladoc Health device (scale, blood glucose meter, etc.) as well as Apple Health data. 

Daily tasks 

As part of the Condition Management program, you will receive three bite-sized daily tasks to complete every 24 hours. Some of these tasks will be specific to your Diabetes Management CGM program. As you complete tasks and progress toward your goals, you’ll earn daily and weekly streaks and badges for reaching these milestones.

*Intended Use:  The Teladoc Health App’s retrospective CGM data analysis function is a mobile-based software feature intended to securely receive retrospective glucose data from compatible third-party continuous glucose monitoring (CGM) systems and their respective applications.
